Compensation:

Position Overview
We are seeking a dedicated Primary Care Physician to join our growing team in Tyler, TX. Our innovative network of neighborhood primary care centers is designed to provide equitable, patient-centered, and value-based healthcare to local Medicare populations.
Our physicians are supported by multidisciplinary care teams and advanced clinical technology, allowing you to focus on what matters most-delivering quality outcomes and preventive care, not patient volume.
Key Responsibilities
- Deliver comprehensive, patient-focused primary care services within a value-based care model.
- Manage a patient panel of approximately 13-16 patients per day.
- Conduct visits averaging 20 minutes for regular appointments and 40 minutes for new or complex cases.
- Collaborate with a dedicated care team including a Medical Assistant, Scribe, and Social Worker.
- Utilize on-site lab services with phlebotomy support and access to RubiconMD for virtual specialty consultations.
- Maintain accurate documentation using Canopy and Greenway EMR systems.
- Participate in community health initiatives such as wellness classes, exercise groups, and social engagement activities.
- Schedule: Monday-Friday, 8:00 AM - 5:00 PM
- Clinic Model: 3-5 providers per site (mix of MDs and NPs)
- Transportation Support: Patient pickup service within 5 miles of the clinic
- Community Engagement: Regular on-site patient wellness events (yoga, dance, spa days, and more)
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.)
- Board Certified in Internal Medicine or Family Medicine
- Active, unrestricted State Medical License and DEA
- Minimum of 1 year post-residency clinical experience (no residents)
- Bilingual (Spanish) preferred
- Geriatrics Fellowship training, preferred
- Base Salary: $215,000-$300,000+ Incentive Bonuses
